The Fire Department of the City of New York (FDNY) provides the public with access to certain agency records in compliance with City, State, and Federal laws.

You can request a:

  • Fire Incident Report that shows the details of a response to a fire or other emergency incident at a particular location.
  • Pre-Hospital Care Report that shows the details of a patient’s treatment and/or emergency transportation.

The FDNY creates reports on its response to fire and other emergency incidents (including non-fire emergencies) occurring within New York City. The FDNY does not maintain records for incidents occurring outside of the City.

Patients who were treated or transported by FDNY Emergency Medical Services (FDNY EMS) personnel may request a copy of their Pre-Hospital Care Report. These records were previously known as Ambulance Call Reports.

The FDNY only maintains Pre-Hospital Care Reports for patients treated or transported by FDNY EMS.

For all other patient records, contact the responding ambulance provider or receiving hospital. If an invoice was issued for emergency transportation, it will list the name of the ambulance provider. If the provider is not the FDNY, you will need to contact the listed provider.
