  • Fire Incident Report that shows the details of a fire emergency response at a particular location
  • Emergency Medical Service Ambulance Report that shows the details of an ambulance treatment record

You can also get forms and instructions about open enforcement actions or request copies of fire reports online. 

The FDNY creates Fire Incident Reports (NYFIR) for an emergency response occurring within New York City. The FDNY does not maintain copies of the fire incident records for emergency responses occurring outside of the City.

Patients who are treated by New York City Fire Department Emergency Medical Service (FDNY EMS) personnel may request a copy of their ambulance treatment record, also known as a Pre-hospital Care Report (PCR).

FDNY EMS only maintains PCRs for patients treated and transported to the hospital by FDNY EMS personnel.

Voluntary Hospital Ambulance Providers PCRs

FDNY EMS does not maintain copies of Pre-hospital Care Reports (PCRs) for patients transported by voluntary hospital ambulance providers (i.e. Mt. Sinai EMS, Northwell EMS, NYU EMS, Presbyterian EMS, etc.).   

If you were treated and/or transported to the hospital by a voluntary hospital ambulance provider, you must contact that provider directly to obtain a copy of your PCR.
