You can make a complaint about plain bottled water to the New York State Department of Health (NYSDOH).

You will need to provide:

  • Complaint details, including pictures if possible
  • Contact information on the bottled water label
  • The NYSHD Certification number on the label (3 digit number)
  • Bottle volume size and pack size it came from, if applicable
  • Any alphanumerical information on the label or bottle itself (eg. best by dates, production stamps)
  • Date, time and location where the water was purchased 
    • If purchased by a delivery service, provide all information on the distributor you have available
  • Number of unopened bottles from the same batch, if applicable
  • Any other information you would like to have on record

Make sure to provide a daytime phone number or email address so that NYSDOH can follow-up on your complaint.

Save and refrigerate the remaining water in its bottle and any other bottles you bought at the same time.
