Prescription Formula Preparation Permit

You need a Prescription Formula Preparation Permit to sell prescription formula.

Fees

If you apply between:

October 1 and March 31, the fee is $175
April 1 and September 30, the fee is $350

The permit renewal fee is $350 per year.

Expiration Date

The permit expires each year on March 31.

Renew by Mail

Several weeks before your permit expires, the Department of Health and Mental Hygiene (DOHMH) will mail you a renewal package. Follow the directions on the renewal notice to renew by mail.
