When you file a Service Request (SR) with NYC311, it is automatically forwarded to the appropriate City Agency for investigation and resolution.

NYC311 sends survey offers when the Agency closes SRs. Survey offers are emailed or texted using the contact information provided in the SR. Anonymous SRs do not receive offers.

Please note that this survey data is used to broadly measure and improve City services, not to address individual SRs.

If the SR you're providing feedback about has been closed but the issue remains unresolved, you should also refile the SR.
